Forging a New Path...
Jefferson Trail offers intensive, child-centered, family-focused treatment in a safe and secure environment to adolescent males and females ages 13 to 17. We work with each child, his or her family members, and step-down placement programs in order to achieve successful transition after Jefferson Trail.
Treatment Approach
The Jefferson Trail team uses a multi-disciplinary approach to develop the strengths of both child and family. Our Assessment & Diagnostic program includes both child and family in individualized assessments. Our Substance Abuse Treatment program is built on evidence-based practices. Each child in each program has an individual treatment plan and a proposed discharge plan implemented upon admission.
Length of Stay
Length of stay varies according to each child’s progression toward his or her treatment goals, with the exception of the Assessment & Diagnostic Program and the Substance Abuse Treatment Program, both of which have targeted short-term stays.
Family Support
Children have weekly phone contact and family therapy sessions. Let us know if we can provide assistance with transportation for visits and family sessions.
After Treatment
Jefferson Trail works closely with community partners to locate foster families and step-down placements when needed.
